Our practice is ready to help you get better and back to work after an injury on the job.
Worker’s Compensation claims require unique handling. If all the correct information is in place from the start, there won’t be any delays in care (ordering MRI’s, surgery if needed…).
New Worker’s Comp rules as of 12/1/10 rely on Medical Treatment Guidelines. Visit the NYS WCB Website for additional info and to download their forms.
What to do:
- Notify your employer, in writing: when, where and how you were injured.
- Fill out a WCB C-3 form.
- If you injured the same body part before, or had treatment for this body part before, you must also file a Form C-3.3.
- Send the form to the WCB; keep a copy for yourself.
How to get medical treatment:
- If your employer’s WC insurer has a Preferred Provider Organization (PPO), you must get your initial treatment from those providers. The PPO may also specify a pharmacy or place to get XRs/MRI’s, etc… Your employer’s WC insurer must tell you about these providers and how to use them.
- Otherwise, you can choose any doctor authorized by the WC Board or occupational health clinics.
