Medical Records Request

We use an electronic medical record by eClinicalWorks so all staff have immediate, secure access to your chart whenever you call.  Notes from your visit are faxed to your Primary Care Physician within 2 days, ensuring that the right clinicians are informed of your progress.  With your permission, we use the RHIO to retrieve MRI and other test results.

If you need a copy of your medical records, this is our process:

Call the office and tell us:

  • Who requested records
  • What records are needed (ie: Worker’s Comp Right Shoulder issue…)
  • Where, when and how they are to be sent.
  • Records that require the patient to sign a release form:
    • All attorney requests. The authorization needs to be up to date, specifying the information/records to be sent and the dates of service start dates
    • All middleman companies handling disability forms/records/payments for another organization.  
  • Records that do NOT require the patient to sign a release form:
    • Physician-office requests dealing with treatment
    • Patients request that their own records be sent to another provider.
    • Insurance company requests where the company is paying for services rendered by our office (ie: Medicare, Excellus, etc…)
  • Records requested by physicians and patients are faxed to the recipient within 2 business days.  Printed copies can be made for $0.75/page + postage.
  • Records requested by attorneys are printed, tallied ($0.75/page + postage) and invoice mailed to the attorney.  Once payment is received, the records are mailed.
