A) Referrals and Second Opinions

I’ve intentionally focused my practice on elective, outpatient orthopaedic care of the extremities. So, your patients can rely on our office for nearly all of their bone and joint treatment.  (Exceptions:  joint replacements, back pain/surgery, major trauma , pediatric care, Fibromyalgia, rheumatoid arthritis and similar conditions.)

Our schedule is predictable and allows time for individualized care. You’ll receive notes about each patient’s visit within 1-2 days, and I’m always available for questions.

X-rays of most extremities can be taken with our digital machine at their appointment, saving an extra stop and co-pay, when possible.

B) Worker’s Comp

Whether or not your office participates in Worker’s Comp, we can help you!

Worker’s Compensation claims require unique handling. Our practice has mastered the paperwork to keep the process on track.

We gather all the correct information from the start so there won’t be any delays in care (ordering MRI’s, surgery if needed…) and returning the patient to work.

New Worker’s Comp rules as of 12/1/10 rely on Medical Treatment Guidelines. Visit the NYS WCB Website for additional info.

What to do:

  • If it’s an acute, urgent injury, we’ll handle the process with a phone call.
  • The patient must complete a WCB C-3 form.
    • If they’ve injured the same body part before, or had treatment for this body part before, they must also file a Form C-3.3.
  • Employer must complete a WCB C-2 form.
  • Patients sends the form to the WCB; they should keep a copy 

How to get medical treatment:

  • If their employer’s WC insurer has a Preferred Provider Organization (PPO), the patient must get initial treatment from those providers.  The PPO may also specify a pharmacy or place to get XRs/MRI’s, etc…  The WC insurer must tell them about these providers and how to use them.
  • Otherwise, they can choose any doctor authorized by the WC Board.
  • Our practice is ready to help your patient get better and back to work after an injury on the job.
  • For an acute, urgent Orthopaedic Extremity injury, we’ll handle the process with a phone call: 585-271-4272.
  • For other Orthopaedic Extremity injuries:  The patient completes our WC Claim Verification Form for patient, incident and company contact info.
    • For a minor injury (needs up to 2 doctor’s visits and up to 1 day lost time from work), the employer may choose to pay for the first aid treatments directly.

  • Patient sends a copy of BOTH the C-3 form and WC Claim Verification Form to our office:
    • Fax to 585-730-6936, or
    • Mail or drop off at our office: 980 Westfall Rd., Ste. 105, Rochester, NY 14618.
  • The claim will be verified with the insurance adjuster and we’ll contact the patient to schedule an appointment within 1-3 days.  They’ll be on the road to recovery!
